The home and community support services agency must complete and return the Individual Service Plan (ISP) attachments and the medical necessity and level of care assessment to the case manager's office during the second month before the month the ISP expires, according to reassessment due dates listed in Appendix XIX of the Community Based Alternatives Provider Manual (CBA Reassessment Packet Due Dates).
